For management of a high INR due to Warfarin:

  • See the Red Cross Guidelines
  • Note these guidelines refer to patients with High and Low bleeding risk
    • This is based on this MJA Article 2013 and refers to High bleeding risk as having any risk factors for major bleeding which include:
      • Major bleeding within previous 4 weeks
      • Surgery within the previous 2 weeks
      • Platelet count less than 50 x 109/L
      • Known liver disease
      • Concurrent antiplatelet therapy
